Please read this consent carefully before starting treatment. By completing an intake, consulting a provider, or receiving treatment through Science & Co, you confirm that you have read and understood this consent and that you agree to receive care by telehealth. This consent forms part of our Terms & Conditions.
Telehealth means receiving healthcare from a licensed provider remotely, using technology rather than an in-person visit. Through Science & Co this generally takes the form of an asynchronous consultation, meaning you complete an intake questionnaire and share information about your health, and a licensed provider reviews it and responds. It may also involve secure messaging, telephone or video contact.
The provider who reviews your information is an independent, licensed healthcare professional, licensed in the state in which you are located. Science & Co is a technology platform. It does not practise medicine and does not make clinical decisions.
Telehealth is not identical to an in-person visit, and it carries limitations you should understand before consenting.
Where clinically appropriate, a provider may prescribe a compounded medication. Compounded drug products are not approved or evaluated for safety, effectiveness or quality by the FDA. Compounding pharmacies are licensed and inspected by state Boards of Pharmacy and are highly regulated, but the FDA has not evaluated these preparations. If you have questions about compounded medication, raise them with your provider before starting treatment.
You are not obliged to use telehealth. You may choose to see a provider in person instead, and you may seek a second opinion at any time. If you would prefer in-person care, or if your provider recommends it, you should arrange it.
Telehealth is not for emergencies. Do not use this service, secure messaging or text messages to report a medical emergency. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. If you are having thoughts of harming yourself, call or text 988 to reach the Suicide and Crisis Lifeline.
Your consultation, the information you provide and any resulting prescription form part of your medical record, held by your provider. Those records are protected under HIPAA and applicable state law, and are described in our HIPAA Notice. Information that is not part of your medical record is handled under our Privacy Policy.
You have the right to access your records and to request that they be shared with another provider. Contact support@science.co and we will direct your request appropriately.
By consenting to telehealth you agree that your provider and our care team may contact you about your care by email, secure message, telephone and text message. Messages relating to your treatment are necessary in order to deliver care. Marketing messages are separate and optional, and are described in our SMS Privacy notice.
The Services are provided on a cash pay basis. Neither Science & Co nor the providers and pharmacies you access through the Services submit claims to Medicare, Medicaid or any private insurer on your behalf. You are responsible for the cost of your consultation and any medication prescribed. Pricing is shown before you pay.
You may withdraw this consent at any time by contacting us at support@science.co. Withdrawing consent stops future telehealth care through Science & Co. It does not affect care already provided, and it does not remove records your provider is required to retain by law.
