Telehealth Consent

Please read this consent carefully before starting treatment. By completing an intake, consulting a provider, or receiving treatment through Science & Co, you confirm that you have read and understood this consent and that you agree to receive care by telehealth. This consent forms part of our Terms & Conditions.

What Telehealth Is

Telehealth means receiving healthcare from a licensed provider remotely, using technology rather than an in-person visit. Through Science & Co this generally takes the form of an asynchronous consultation, meaning you complete an intake questionnaire and share information about your health, and a licensed provider reviews it and responds. It may also involve secure messaging, telephone or video contact.

The provider who reviews your information is an independent, licensed healthcare professional, licensed in the state in which you are located. Science & Co is a technology platform. It does not practise medicine and does not make clinical decisions.

Possible Benefits

  • Access to a licensed provider without travelling to a clinic or waiting for an appointment.
  • Care that fits around your schedule, including outside typical clinic hours.
  • Medication shipped directly to you where a provider determines it is appropriate.
  • Continuity of care and ongoing follow up with a provider.

Limitations and Risks

Telehealth is not identical to an in-person visit, and it carries limitations you should understand before consenting.

  • No physical examination. Your provider will not physically examine you. They rely on the information you give them. A condition that would be detected during an in-person examination may not be identified.
  • Your information must be accurate. Because the assessment rests on what you report, incomplete or inaccurate information could lead to an inappropriate treatment decision, an adverse reaction, or a missed diagnosis.
  • Not suitable for every condition. Some conditions cannot safely be assessed or treated remotely. Your provider may determine that telehealth is not appropriate for you and may decline to prescribe or may refer you for in-person care.
  • Technology can fail. Delays, outages, poor connectivity or equipment failure can interrupt or delay care.
  • Security. We use reasonable safeguards to protect your information, but no electronic transmission or storage is completely secure. In rare cases a breach could occur.
  • Treatment is not guaranteed. Completing an intake or paying does not guarantee that a prescription will be issued. Prescribing decisions rest solely with the provider, based on their clinical judgement.
  • Outcomes vary. No provider can guarantee a particular result from any treatment. Individual results differ.
  • Side effects. All medications carry a risk of side effects. Your provider will discuss the risks relevant to any treatment recommended for you, and you should read the information supplied with your medication.

Compounded Medications

Where clinically appropriate, a provider may prescribe a compounded medication. Compounded drug products are not approved or evaluated for safety, effectiveness or quality by the FDA. Compounding pharmacies are licensed and inspected by state Boards of Pharmacy and are highly regulated, but the FDA has not evaluated these preparations. If you have questions about compounded medication, raise them with your provider before starting treatment.

Your Alternatives

You are not obliged to use telehealth. You may choose to see a provider in person instead, and you may seek a second opinion at any time. If you would prefer in-person care, or if your provider recommends it, you should arrange it.

Emergencies

Telehealth is not for emergencies. Do not use this service, secure messaging or text messages to report a medical emergency. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. If you are having thoughts of harming yourself, call or text 988 to reach the Suicide and Crisis Lifeline.

Your Health Records

Your consultation, the information you provide and any resulting prescription form part of your medical record, held by your provider. Those records are protected under HIPAA and applicable state law, and are described in our HIPAA Notice. Information that is not part of your medical record is handled under our Privacy Policy.

You have the right to access your records and to request that they be shared with another provider. Contact support@science.co and we will direct your request appropriately.

Communications

By consenting to telehealth you agree that your provider and our care team may contact you about your care by email, secure message, telephone and text message. Messages relating to your treatment are necessary in order to deliver care. Marketing messages are separate and optional, and are described in our SMS Privacy notice.

Costs

The Services are provided on a cash pay basis. Neither Science & Co nor the providers and pharmacies you access through the Services submit claims to Medicare, Medicaid or any private insurer on your behalf. You are responsible for the cost of your consultation and any medication prescribed. Pricing is shown before you pay.

Withdrawing Your Consent

You may withdraw this consent at any time by contacting us at support@science.co. Withdrawing consent stops future telehealth care through Science & Co. It does not affect care already provided, and it does not remove records your provider is required to retain by law.

Your Consent

By continuing, you confirm that:

  • you are at least eighteen years old and are located in the United States;
  • you have read and understood this consent, including the limitations and risks described above;
  • you have had the opportunity to ask questions, and understand you can ask more at any time;
  • the information you provide will be true, accurate and complete to the best of your knowledge;
  • you understand that no specific outcome is guaranteed and that a prescription may not be issued; and
  • you consent to receive care by telehealth from an independent, licensed provider.
